Every patient who receives health care services at the UHSM has a medical record. The medical record is necessary to document the care and services provided to you and to insure a continuity of care.
Accessing Your Medical Records
All medical records are handled with strict confidentiality. Access is limited to authorized personnel only. Your written consent is required to release any information, except as permitted by law or court order.
