Monitored Study Session Form

This form is used to document study time completed in a monitored or approved setting. Students may use it to verify study sessions, tutoring, academic support, computer lab time, or other education-related activities that are supervised or confirmed by an approved staff member.

Complete the form as instructed, have the study session verified when required, save a copy for your records, and submit it by the required deadline to help document your First-to-Work educational participation hours.
