Medical Records
Every patient who receives health care services at the Leeward Community College Student Health Center has a medical record. The medical record is necessary to document the care and services provided to you and to ensure a continuity of care.
Accessing Your Medical Records
All medical records are treated in a strictly confidential manner. Your written authorization is necessary to release any information, except when required by law or a court order.
Important Notice: Effective July 1, 2026, due to a change in our health record system, how you access your records depends on the date of your visit. Note: There may be a nominal charge for processing and duplicating these records.
For Care Received Before July 1, 2026
University Health Services Mānoa (UHSM) will retain access to these medical records. To request a copy of your records from a visit prior to this date:
- Complete and sign the UHSM Consent for Release of Medical Information or Records Form (PDF)
. - Submit the completed form to University Health Services Mānoa.
For Care Received On or After July 1, 2026
- Complete the Leeward CC Consent for Release of Medical Information or Records Form (PDF)
