  {"id":37612,"date":"2015-08-20T08:54:39","date_gmt":"2015-08-20T18:54:39","guid":{"rendered":"http:\/\/www.hawaii.edu\/news\/?p=37612"},"modified":"2021-08-05T16:26:42","modified_gmt":"2021-08-06T02:26:42","slug":"model-program-expands-role-of-pharmacists-in-tracking-high-risk-patients","status":"publish","type":"post","link":"https:\/\/www.hawaii.edu\/news\/2015\/08\/20\/model-program-expands-role-of-pharmacists-in-tracking-high-risk-patients\/","title":{"rendered":"Model program expands role of pharmacists in tracking high risk patients"},"content":{"rendered":"<span class=\"span-reading-time rt-reading-time\" style=\"display: block;\"><span class=\"rt-label rt-prefix\">Reading time: <\/span> <span class=\"rt-time\"> 3<\/span> <span class=\"rt-label rt-postfix\">minutes<\/span><\/span><p>The role of pharmacists in <span aria-label=\"Hawaii\">Âé¶¹´«Ã½<\/span> communities is being redefined in an innovative healthcare program administered by the <a href=\"http:\/\/hilo.hawaii.edu\/\">University of <span aria-label=\"Hawaii\">Âé¶¹´«Ã½<\/span> at Hilo<\/a>. Traditionally, pharmacists are isolated dispensers of medication, located in stationary storefronts. But in the Pharm2Pharm program, carried out over the past three years in select areas throughout the state, pharmacists play a much more integrated and proactive role in overseeing high risk patients&#8217; medication routines.<\/p>\n<p>&ldquo;Patients who are on a lot of medications or certain types of medications are at higher risk for problems that could land them in the hospital,&rdquo; says <strong>Karen Pellegrin, director of continuing and distance education and strategic planning at the <a href=\"http:\/\/pharmacy.uhh.hawaii.edu\/\">Daniel K. Inouye College of Pharmacy<\/a> at <abbr>UH<\/abbr> Hilo and principal investigator of the Pharm2Pharm program.<\/strong><\/p>\n<p>&ldquo;Through the Pharm2Pharm project, we&#8217;ve shown that consulting pharmacists\u2014those who coordinate medications across prescribers and across dispensing pharmacies\u2014can help make sure patients are on the right medications and are taking them properly,&rdquo; she says. &ldquo;These pharmacists have made a difference statewide, but especially in our rural communities where physician shortages are most severe.&rdquo;<\/p>\n<p>Problems with medication often arise for patients due to lack of coordination between prescribers, inadequate monitoring, and misunderstanding about how to take them properly. &ldquo;Consulting pharmacists,&rdquo; as they are called in the program, track patients\u2019 use of prescription medication as they transition from hospital to home, a high-risk period for medication problems.<\/p>\n<p>The consulting pharmacists meet with patients regularly and utilize secure computer software to monitor a patient&#8217;s overall medication plan and network with others on the patient&#8217;s healthcare team. This is the first time in <span aria-label=\"Hawaii\">Âé¶¹´«Ã½<\/span> that information technology has been implemented on a statewide basis for medication management. The <a href=\"https:\/\/www.hawaiihie.org\/\"><span aria-label=\"Hawaii\">Âé¶¹´«Ã½<\/span> Health Information Exchange<\/a> is providing the information technology infrastructure to support the program.<\/p>\n<h2>Positive responses from patients<\/h2>\n<p>The feedback from patients is outstanding. Many patients participating in Pharm2Pharm report the services have helped keep them out of the hospital and improve their overall health and well-being.<\/p>\n<p>&ldquo;This program has changed my life,&rdquo; reports one patient. &ldquo;I have returned to my old self; I can sleep at night, fear of never waking is gone. No more inhaler and wheezing. Not so many pills and knowing when or when not to take them also the security of knowing I have someone to talk with when I have a question or problem. Thank you for my life back, and I really mean this.&rdquo;<\/p>\n<p>Another patient says, &ldquo;My pharmacist was extremely helpful beyond my expectations.&rdquo;<\/p>\n<p>Pellegrin says the positive results are &ldquo;what happens when pharmacists become part of the care team.&rdquo;<\/p>\n<h2>Collaboration<\/h2>\n<p><a href=\"http:\/\/www.communitypharmacist.org\/\"><span aria-label=\"Hawaii\">Âé¶¹´«Ã½<\/span> Community Pharmacists Association<\/a> members have provided the outpatient services. <a href=\"https:\/\/www.hawaiipacifichealth.org\/\"><span aria-label=\"Hawaii\">Âé¶¹´«Ã½<\/span> Pacific Health<\/a> and <a href=\"http:\/\/www.hhsc.org\/\"><span aria-label=\"Hawaii\">Âé¶¹´«Ã½<\/span> Health Systems Corporation<\/a> also are collaborating partners in Pharm2Pharm, enrolling patients from several of their hospitals. In all, seven hospitals in the state have participated in Pharm2Pharm. Pellegrin says the final count of patients receiving Pharm2Pharm services is 2,570 statewide, most in rural areas.<\/p>\n<p>The pharmacy college was awarded $14.3 million in 2012 to develop the program to save health care costs, one of 107 awardees competing successfully against more than 3,000 other highly qualified applicants for the funding. The program is funded by the <a href=\"https:\/\/www.cms.gov\/\">Centers for Medicare and Medicaid Services<\/a> Innovation Center.<\/p>\n<p>Pharm2Pharm is now in its fourth and final no-cost extension year focusing on evaluation and sustainability.<\/p>\n<p>Pellegrin says since the launch of <abbr>UH<\/abbr> Hilo&#8217;s pharmacy college, the supply of pharmacists in <span aria-label=\"Hawaii\">Âé¶¹´«Ã½<\/span> has greatly improved.
