Inouye College of Pharmacy student<\/figcaption><\/figure>\n<p>Pharmacists on <span aria-label=\"Oahu\">O&#699;ahu<\/span> are now helping patients at risk of medication-related hospitalization readmissions through Pharm2Pharm, a project from the <a href=\"http:\/\/pharmacy.uhh.hawaii.edu\/\">Daniel K. Inouye College of Pharmacy<\/a> (<abbr>DKICP<\/abbr>) at the <a href=\"http:\/\/hilo.hawaii.edu\/\">University of <span aria-label=\"Hawaii\">Âé¶¹´«Ã½<\/span> at Hilo<\/a>.<\/p>\n<p>Patients leaving <a href=\"https:\/\/www.palimomi.org\/\">Pali Momi Medical Center<\/a> in West <span aria-label=\"Oahu\">O&#699;ahu<\/span> and <a href=\"https:\/\/www.adventisthealth.org\/castle\/pages\/castle-home.aspx\">Castle Medical Center<\/a> in Windward <span aria-label=\"Oahu\">O&#699;ahu<\/span> can take advantage of the program during their recovery.<\/p>\n<p>&ldquo;The hospital consulting pharmacists (<abbr>HCP<\/abbr>s) use risk-based criteria to identify inpatients who may benefit from further care once they leave the hospital,&rdquo; said <strong>Karen Pellegrin<\/strong>, director of the project based in <abbr>DKICP<\/abbr>&#8217;s Center for Rural Health Science. &ldquo;The <abbr>HCP<\/abbr>s also consider referrals from community docs who have high-risk patients not currently in the hospital who might benefit.&rdquo;<\/p>\n<p><abbr>HCP<\/abbr>&#8217;s <strong>Angela Li<\/strong>, <strong>Shanele Shimabuku<\/strong> and <strong>Elizabeth Lee<\/strong>, who are all graduates of <abbr>DKICP<\/abbr> PharmD program, identify high-risk patients and transfer them to the care of community consulting pharmacists after discharge. Shimabuku completed a pharmacy residency through the college working with Senior Clinical Pharmacist and <abbr>DKICP<\/abbr> Assistant Professor <strong>Roy Goo<\/strong>, who is based on <span aria-label=\"Kauai\">Kaua&#699;i<\/span>.<\/p>\n<p>&ldquo;The launch on <span aria-label=\"Oahu\">O&#699;ahu<\/span> was timed with the implementation of state-of-the-art health information technology by <span aria-label=\"Hawaii\">Âé¶¹´«Ã½<\/span> Health Information Exchange (<abbr>HHIE<\/abbr>), a key project support partner,&rdquo; said Pellegrin. &ldquo;The <span aria-label=\"Hawaii\">Âé¶¹´«Ã½<\/span> Commnity Pharmacist Association and <span aria-label=\"Hawaii\">Âé¶¹´«Ã½<\/span> Pacific Health also are key operating partners for the Pharm2Pharm project, which is funded through the federal Center for Medicare and Medicaid Innovation within the Centers for Medicare and Medicaid Services (<abbr>CMS<\/abbr>).&rdquo;<\/p>\n<p>With wireless mobile devices, consulting pharmacists can access cloud-based prescription-fill histories on their patients and ensure a complete and accurate medication list is available to other members of the care team through use of Health Care Systems, Inc. medication management tools. Participating consulting pharmacists can also communicate with each other and with other members of the care team through <abbr>HHIE<\/abbr>&#8217;s secure messaging system.<\/p>\n<p>Pellegrin said they are currently focusing on hospitals in regions of <span aria-label=\"Oahu\">O&#699;ahu<\/span> with higher rates of potentially preventable hospitalization rates. More than 1,300 high risk patients have been enrolled statewide since Pharm2Pharm was implemented on Maui, <span aria-label=\"Kauai\">Kaua&#699;i<\/span> and <span aria-label=\"Hawaii\">Âé¶¹´«Ã½<\/span> Island in 2013.<\/p>\n<p><em>&#8212;By Alyson Kakugawa-Leong<\/em><\/p>\n","protected":false},"excerpt":{"rendered":"<p>The <abbr>UH<\/abbr> Hilo Daniel K.
