A parent\/guardian is required to sign for patients under 18 years of age.<\/li>\n<li>Bring a valid government issued photo <abbr title=\"identification\">ID<\/abbr><\/li>\n<li>If you have medical insurance, bring your insurance card. A vaccine that requires a two-dose regimen will be provided on the following dates:<\/p>\n<ul>\n<li>Thursday, August 19 (first dose), 12&#8211;5 p.m. at the <abbr>UH<\/abbr> Hilo Hale Kehau residence hall lounge<\/li>\n<li>The date, time and location for the second dose will be provided to the students who received the first dose<\/li>\n<\/ul>\n<p>Participants can <a href=\"https:\/\/docs.google.com\/forms\/d\/e\/1FAIpQLSfSp8pMQBqGGtr7r5PeKmL0vvSX-pRBnUeFkabtPXeuolMMrA\/viewform\">pre-register online<\/a>, and vaccinations are only available to <abbr>UH<\/abbr> Hilo students.<\/p>\n<p>Participants are required to:<\/p>\n<ul>\n<li>Receive clearance from <a href=\"https:\/\/uh.campus.lumisight.com\/\">LumiSight <abbr>UH<\/abbr><\/a> health check app prior to coming to campus<\/li>\n<li>Bring a valid government issued photo <abbr>ID<\/abbr><\/li>\n<li>If you have medical insurance, bring your insurance card. Uninsured participants are welcome.<\/li>\n<li>Please wear a mask and practice social distancing<\/li>\n<\/ul>\n<p>Participants will be considered fully vaccinated two weeks after their second shot.<\/p>\n<h2><abbr>UH<\/abbr> West <span aria-label=\"Oahu\">O&#699;ahu<\/span><\/h2>\n<p><abbr>UH<\/abbr> West <span aria-label=\"Oahu\">O&#699;ahu<\/span> will host a mobile vaccination clinic that is open to Â鶹´«Ã½students, employees and the general public.<\/p>\n<p>The Pfizer <abbr>COVID-19<\/abbr> vaccine, requiring a two-dose regimen, will be provided on the following dates:<\/p>\n<ul>\n<li>Monday, August 23, (first dose), 7:30 a.m.&#8211;3 p.m. at the <span aria-label=\"Waianae\">Wai&#699;anae<\/span> side of the campus expansion parking lot.<\/li>\n<li>September 13 (first dose or second dose for those who were vaccinated on August 23), 7:30 a.m.&#8211;3 p.m. at the same location<\/li>\n<li>October 4 (second dose for those who received their first dose on September 13), 7:30 a.m.&#8211;3 p.m. at the same location<\/li>\n<\/ul>\n<p>Walk-ins are welcome (no appointment needed) or participants can <a href=\"https:\/\/docs.google.com\/forms\/d\/e\/1FAIpQLScA40Q99rLKAPJ0JiNArRGrRuzoAzPJXB9gLvZDZugEgyZYjQ\/viewform\">pre-register online<\/a>. Uninsured participants are welcome.<\/li>\n<li>Wear a mask and practice social distancing<\/li>\n<\/ul>\n<h2>Get Vaccinated <abbr>UH<\/abbr>!<\/h2>\n<p><abbr>UH<\/abbr> launched its <a href=\"https:\/\/www.hawaii.edu\/news\/2021\/03\/05\/10-campuses-encouraged-to-get-vaccinated-uh\/\">&ldquo;Get Vaccinated <abbr>UH<\/abbr>!&rdquo; campaign<\/a> back on March 5, encouraging students, faculty and staff to get a <abbr>COVID-19<\/abbr> vaccine. The university added <a href=\"https:\/\/www.hawaii.edu\/news\/2021\/05\/17\/uh-to-require-covid-19-vaccination\/\"><abbr>COVID-19<\/abbr> vaccination to its student health clearance requirements<\/a> on May 17. For the fall semester, unvaccinated students are required to submit a negative <abbr>COVID-19<\/abbr> test each week if they plan to be on a Â鶹´«Ã½campus.
