  {"id":127637,"date":"2020-09-25T11:47:48","date_gmt":"2020-09-25T21:47:48","guid":{"rendered":"https:\/\/www.hawaii.edu\/news\/?p=127637"},"modified":"2020-09-25T11:48:39","modified_gmt":"2020-09-25T21:48:39","slug":"hawaii-telemedicine-survey-results","status":"publish","type":"post","link":"https:\/\/www.hawaii.edu\/news\/2020\/09\/25\/hawaii-telemedicine-survey-results\/","title":{"rendered":"Survey finds Hawai\u02bbi telemedicine up, obstacles remain"},"content":{"rendered":"<span class=\"span-reading-time rt-reading-time\" style=\"display: block;\"><span class=\"rt-label rt-prefix\">Reading time: <\/span> <span class=\"rt-time\"> 3<\/span> <span class=\"rt-label rt-postfix\">minutes<\/span><\/span><p><img loading=\"lazy\" decoding=\"async\" src=\"https:\/\/www.hawaii.edu\/news\/wp-content\/uploads\/2020\/09\/system-telehealth.jpg\" alt=\"female doctor on telehealth session\" width=\"676\" height=\"381\" class=\"aligncenter size-full wp-image-127661\" srcset=\"https:\/\/www.hawaii.edu\/news\/wp-content\/uploads\/2020\/09\/system-telehealth.jpg 676w, https:\/\/www.hawaii.edu\/news\/wp-content\/uploads\/2020\/09\/system-telehealth-300x169.jpg 300w, https:\/\/www.hawaii.edu\/news\/wp-content\/uploads\/2020\/09\/system-telehealth-130x73.jpg 130w\" sizes=\"auto, (max-width: 676px) 100vw, 676px\" \/><\/p>\n<p><span aria-label=\"Hawaii\">Âé¶¹´«Ã½<\/span> healthcare providers have ramped up their capacity to meet the new demand for telemedicine due to <abbr title=\"coronavirus 2019\">COVID-19<\/abbr>, according to <a href=\"https:\/\/uhealthy.hawaii.edu\/telemedicine-needs-assessment\/\">results of a statewide telemedicine assessment<\/a> conducted by the <a href=\"https:\/\/dod.hawaii.gov\/hiema\/\"><span aria-label=\"Hawaii\">Âé¶¹´«Ã½<\/span> Emergency Management Agency<\/a> (<abbr>HI-EMA<\/abbr>) in collaboration with the <a href=\"https:\/\/www.hawaii.edu\/\">University of <span aria-label=\"Hawaii\">Âé¶¹´«Ã½<\/span><\/a>. <\/p>\n<p>&ldquo;We will be living with <abbr>COVID-19<\/abbr> for an unknown period of time and one of the challenges our agency faces is how to mitigate against hospital capacity, and from an emergency management perspective, telemedicine is a key component in our battle with this pandemic,&rdquo; said <abbr>HI-EMA<\/abbr> Administrator Luke Meyers.<\/p>\n<p>Since health care in the time of <abbr>COVID-19<\/abbr> has increasingly shifted from in-person to virtual, <abbr>HI-EMA<\/abbr>\u2019s State Emergency Support Function &#35;8 (<abbr>SESF<\/abbr> #8), which is responsible for medicine and public health, decided to examine telemedicine readiness and needs in the state of <span aria-label=\"Hawaii\">Âé¶¹´«Ã½<\/span>.<\/p>\n<p>&ldquo;This survey is the only known statewide telemedicine assessment in <span aria-label=\"Hawaii\">Âé¶¹´«Ã½<\/span>,\u201d said <strong>Aimee Grace<\/strong>, the telehealth component lead for <abbr>HI-EMA<\/abbr>\u2019s <abbr>SESF<\/abbr> #8 and director of the <abbr title=\"University of Hawaii\">UH<\/abbr> System\u2019s <a href=\"https:\/\/www.hawaii.edu\/research\/about-uhealthy\/\">Office of Strategic Health Initiatives<\/a>. &ldquo;The results are helping us to better understand the capacity and needs in the state with regard to telemedicine as we work together to address <abbr>COVID-19<\/abbr>.&rdquo; <\/p>\n<p>Conducted in May 2020, <abbr>HI-EMA<\/abbr>\u2019s statewide telemedicine needs assessment received more than 100 responses across the state from private and public hospitals, Federally Qualified Health Centers, rural health clinics, independent providers, Native Hawaiian Health Care Systems and more. The majority of the respondents reported that they recently began providing virtual patient visits and tele-triage services.<\/p>\n<p>&ldquo;The study indicated that while <span aria-label=\"Hawaii\">Âé¶¹´«Ã½<\/span> has a significant capacity for telemedicine in some areas, large gaps remain in others,&rdquo; said <strong>Lee Buenconsejo-Lum<\/strong>, associate dean for academic affairs at <abbr>UH<\/abbr> M\u0101noa\u2019s <a href=\"http:\/\/jabsom.hawaii.edu\">John A. Burns School of Medicine<\/a> (<abbr>JABSOM<\/abbr>), a member of the survey core team and SESF &#35;8. &ldquo;For example, while some telemedicine services such as telestroke, tele-triage and asynchronous store-and-forward technology could be found statewide prior to <abbr>COVID-19<\/abbr>, it was not until after the onset of the pandemic that these telemedicine services became a major presence in different counties and facilities.&rdquo;<\/p>\n<p>Many respondents stated their desire to start or expand telemedicine services, such as remote patient monitoring, eConsults, asynchronous store and forward technology, and real-time imaging. While specific preferences for desired services varied by facility or practice type, counties across the state of <span aria-label=\"Hawaii\">Âé¶¹´«Ã½<\/span> desired numerous types of telemedicine services.<\/p>\n<h2>Obstacles remain to remote patient care<\/h2>\n<p><img loading=\"lazy\" decoding=\"async\" src=\"https:\/\/www.hawaii.edu\/news\/wp-content\/uploads\/2020\/04\/manoa-pbrtc-telehealth-300x169.jpg\" alt=\"older man looking at cell phone\" width=\"300\" height=\"169\" class=\"alignright size-medium wp-image-116184\" srcset=\"https:\/\/www.hawaii.edu\/news\/wp-content\/uploads\/2020\/04\/manoa-pbrtc-telehealth-300x169.jpg 300w, https:\/\/www.hawaii.edu\/news\/wp-content\/uploads\/2020\/04\/manoa-pbrtc-telehealth-130x73.jpg 130w, https:\/\/www.hawaii.edu\/news\/wp-content\/uploads\/2020\/04\/manoa-pbrtc-telehealth-630x353.jpg 630w, https:\/\/www.hawaii.edu\/news\/wp-content\/uploads\/2020\/04\/manoa-pbrtc-telehealth.jpg 676w\" sizes=\"auto, (max-width: 300px) 100vw, 300px\" \/><\/p>\n<p>While there was wide interest in including telemedicine services, some hospitals and clinics also indicated that they faced barriers including a lack of funding, poor internet connectivity and a lack of staff training. <\/p>\n<p>&ldquo;<abbr>HI-EMA<\/abbr> is very grateful to Dr. Grace and the excellent team that worked so hard to put together this vitally important assessment of our states\u2019 telehealth capacities and needs. The report will not only allow us to improve our ability to deliver medical services across during this pandemic, but it also gives us a path forward to leverage this crisis into an opportunity to improve healthcare delivery for the people of <span aria-label=\"Hawaii\">Âé¶¹´«Ã½<\/span> far into the future,&rdquo; said <strong>Steven Hankins<\/strong>, SESF#8 lead coordinator and a <abbr>JABSOM<\/abbr> family medicine physician.<\/p>\n<p>The survey also showed that, in general, specialized equipment for telemedicine\u2014including telemedicine carts, digital stethoscopes, blood pressure monitors and other peripheral devices\u2014were not commonly found in the state of <span aria-label=\"Hawaii\">Âé¶¹´«Ã½<\/span>. Without such equipment, facilities and practices may not be able to provide desired services such as remote patient monitoring and mobile health. <\/p>\n<h2>Results to help provide path forward<\/h2>\n<p>The results of the survey will be disseminated statewide, providing useful information to state leaders, policymakers, health care facilities and organizations and health professionals, and bolstering upcoming efforts to increase funding for telemedicine and target interventions. Results will also help to inform future telemedicine studies and endeavors. <\/p>\n<p>&ldquo;As we well know, the status of <abbr>COVID-19<\/abbr> in the state of <span aria-label=\"Hawaii\">Âé¶¹´«Ã½<\/span> is ever evolving,&rdquo; said <strong>Christina Higa<\/strong>, co-director of <abbr>UH<\/abbr>\u2019s Pacific Basin Telehealth Resource Center and part of the survey\u2019s core advisory team. &ldquo;This survey was conducted in May, when <span aria-label=\"Hawaii\">Âé¶¹´«Ã½<\/span> was doing well. Our results may already be outdated, and telemedicine may be in even higher demand.
